Pricing: 1 hour individual/couple’s session cost $240
Insurance is not accepted as form of payment but HCFA 1500 or Super Bill forms will be completed on a monthly basis to facilitate submission of services.
Please review the forms below before your first appointment so you can become familiar with my office policies and pricing. You are welcome to bring them completed or you can fill them out before your session. If you are doing remote sessions please complete the forms and email them back to me prior to our first session.
